Reading Interview

The forms below should be used to complete the Reading Interview for each young person. The data from the Reading Profile and Questions About Reading – Pre documents should be entered in the Workflow Management System by the coordinator.

Parental Consent Forms must be completed for young people under the age of 16. It is the responsibility of the coordinator to obtain appropriate consent from parents/carers.
